"Reported violation"

The wording of this sentence is very vague. Was the reported violation taken care of by the anti-harassment team? Did the anti-harassment team take any action against him?
If you toss "direct threats" stories (was he swinging a mace?), you might want to back them up at least with your recollection of the events.
